By signing this document, I hereby acknowledge and agree with the participation of the said participant. I give the consent of physical activities with the youth organization MICHIGAN WARRIORS.Some activities may include physical contact, running, exercise, etc. I have been informed and understand that participation in some activities may expose one to certain foreseeable and unforeseeable risks of damage and/or bodily injury, including serious bodily injury where one may need to be hospitalized. One knowingly, freely, and voluntarily assume all risk in the participation of activities. I hereby release Michigan Warriors from any and all liability arising out the participation of the activities and hereby waive my rights herein to assert any claim(s) for damages, bodily injury, or serious bodily injury to the fullest extent allowed by law. I further agree that I will hold harmless MICHIGAN WARRIORS as well as it's volunteers and/or staff members against any and all claim(s) for damages, bodily injury, or serious bodily injury arising out of or in connection of the participation activities rather caused by negligence or otherwise. I fully understand these terms set forth in this form, and I hereby execute this physical activity release of liability form.
I give permission to travel by motor vehicle with MICHIGAN WARRIORS and its staff and volunteers to and from any and all events and/or activities.
MEDIA RELEASE
I, the parent/ guardian, give consent to MICHIGAN WARRIORS for the release of media to publish, copyright, etc. via images, video, advertisement, internet and/or social media, fundraisers, activities, etc.
